What is Geriatric Care Management?

- In-home assessment with client and loved ones
- Advocating for the individual to help he/she remain independent. Ensuring he/she is treated with dignity, respect and receives all the required services.
- Formulating an appropriate plan of care in conjunction with physicians, and professional caregivers that includes specific recommendations for the individual.
- We will monitor a client's mental/cognitive ability in performing activities of daily living. We will accompany our clients to doctor appointments, and monitor any physical or cognitive changes happening over time.
- Hiring and coordinating a plan for home health aides, 24-hour live ins, medical transportation, housecleaning and meal delivery or meal preparation. In addition, we will assist with ordering durable medical equipment such as canes, walkers, wheelchairs, etc.
- Health care coordination
- Serve as the care "lifeline" between the elderly, and those families separated by distance and/or circumstances.
- Assistance and guidance with exploring and selecting the most appropriate retirement community, assisted living facility, or nursing facility for your loved one.
- Provide phone consultations/email communication with family or loved ones who live at a distance.
- Assistance with navigating the complexities of Medicaid eligibility, and other health insurance-related issues.
- Provide access to local, county, state, and national resources available to the elderly.
- Links to other professions specializing in issues which specifically impact the elderly: attorneys, accountants, bookkeepers, medical specialists, companions, etc.
- Hospice and other "end of life" guidance and counseling.
